Community Navigator Program

How can the Community Navigator Program help me?

The Community Navigator Program (CNP) works directly with you and the regional centers. Our Community Navigators aims to help you and your child utilize community and Regional Center resources. We support families of children with disabilities to get the services they need. 

How do we help?

Our Community Navigators are the bridge between you and your family and community agencies as well as local providers. We support with relationship-based service by:

  1. Providing resources and support materials.
    • We guide you in connecting with or advocate for a specific resource.
  2. Growing your community support to feel a sense of belonging and safety.
    • We assist you with accessing support groups and trainings on requested topics.
  3. Empowering you to self-advocate.
    • Using knowledge of legal rights as well as Triple P, Community Resiliency Model (CRM), etc. to uplift you.
  4. Communicating with Agencies upon your request.
  5. Goal setting and follow up with you consistently until your set goal is met.
    • Planned follow-up calls with you during the process to see if you need further information or other assistance (including helping with forms, making calls on their behalf or attending meetings).
  6. Tracking progress and outcomes by keeping detailed notes of each interaction and share it with Raise & Shine.
